Clinic significance
Anti-ANCA antibodies represent a recently developed marker which is very useful in the diagnosis of vasculitis. Three different staining patterns can be distinguished on polymorphonuclear cells fixed in ethanol and formaldehyde: cytoplasmic, perinuclear and X-ANCA. The two main specificities (myeloperoxidase and proteinase 3) are typed by Flow immunofluorometry. Perinuclear staining due to antinuclear antibodies can interfere with this screen. The presence of anti-PR3 antibodies correlates strongly with Wegener's granulomatosis whereas anti-MPO antibodies are found in crescentic glomerulonephritis and Shurg-Strauss syndrome (although this is less specific).
