Clinic significance

The ZIKA virus is a virus belonging to the Flaviviridae family transmitted to humans by bites of the of the Aeges genus of mosquito (which also transmits dengue and chikungunya)
Asymptomatic in 80% of cases, it may nonetheless be responsible for foetal malformations (microcephaly) during the first semester of pregnancy.
In clinical manifestations in adults, we may observe a strong onset of fever, and itchy skin rash, joint pain or conjunctivitis. The symptoms are generally benign and last less than a week. Neurological complications (Guillain-Barré syndrome) have been reported.
According to curent medical knowledge, PCR is positive in blood between D0 and D5, and in urine up to D10 or even D15 in regard to any clinical signs.
The appearance of antibodies may be observed as of D5 after the onset of any clinical signs but this may be delayed; therefore, in asymptomatic pregnant women and to avoid reporting false negative results, the CNR (Centre national de référence) only carries out testing as of one month after the date of return from the endemic area.
In all cases, serological monitoring is recommended 15 days later.
Serology detects IgG and IgM. Cross-reactivity may exist, in particular with dengue.
Because of the similar geographic distribution and clinical signs, it is recommended to simultaneously test for infection with dengue and chikungunya.
