Purpose
Inhibition of the renin-angiotensin system.
Under the influence of an angiotensin-converting enzyme inhibitor, aldosterone levels drop and renin rises.
For the investigation of hypertension (HT) and aldosteronism, this test differentiates inhibition-resistant primary hyperaldosteronism from hyperplasia and secondary hyperaldosteronism.
Suject preparation
Subject fasting and at rest for at least one hour.
Product
Captopril: Captolane® or Lopril® (25 mg tablets).
Dose
Adults: 1 mg/kg body weight (oral).
Protocol
The subject should be lying down throughout the three hours of the challenge.
- Sample (EDTA tube) at 0 min time interval (T0).
- Oral administration of the correct dose of captopril.
- Sample (EDTA tube) at T+180 min after administration of the product.
Samples
After centrifugation at room temperature, draw off plasma and freeze as soon as possible.
Assays
- Aldosterone.
- Active renin.
Side effects
- Strict medical supervision throughout the challenge with blood pressure measurements every 15 min.
- Risk of hypotension.
Contraindications
- Pregnancy.
- Heart failure.
- Sodium depletion.
