List of dynamic tests
Purpose

Inhibition of the renin-angiotensin system.
Under the influence of an angiotensin-converting enzyme inhibitor, aldosterone levels drop and renin rises.
For the investigation of hypertension (HT) and aldosteronism, this test differentiates inhibition-resistant primary hyperaldosteronism from hyperplasia and secondary hyperaldosteronism.

Suject preparation

Subject fasting and at rest for at least one hour.

Product

Captopril: Captolane® or Lopril® (25 mg tablets).

Dose

Adults: 1 mg/kg body weight (oral).

Protocol

The subject should be lying down throughout the three hours of the challenge.

  • Sample (EDTA tube) at 0 min time interval (T0).
  • Oral administration of the correct dose of captopril.
  • Sample (EDTA tube) at T+180 min after administration of the product.
Samples

After centrifugation at room temperature, draw off plasma and freeze as soon as possible.

Assays
  • Aldosterone.
  • Active renin.
Side effects
  • Strict medical supervision throughout the challenge with blood pressure measurements every 15 min.
  • Risk of hypotension.
Contraindications
  • Pregnancy.
  • Heart failure.
  • Sodium depletion.
