Clinical significance

Synthesized by the adrenal cortex from 17-OHP by the action of 11-beta-hydroxylase. The metabolic pathway is a minor one in normal subjects but assumes importance in those with congenital hyperplasia of the adrenal gland due to 21-hydroxylase deficiency. Complete deficiency is identified at birth in 80% of cases and excess androgens during childhood lead to over-rapid bone development and precocious pseudopuberty. Incomplete deficiency is more common (0,5 to 20% of the population) and is associated with hirsutism, acne vulgaris and alopecia. 11-deoxycortisol levels rise dramatically in a short Synacthen (adrenocortical stimulation) test.
