Anti-myelin antibodies are found in peripheral neuropathies associated with IgM-type monoclonal gammopathy. The latter is usually benign with no associated plasma cell proliferation. These peripheral polyneuropathies are initially sensory, then sensitivomotor, demyelinating and chronic, associated with tremors in the classic form. Anti-myelin activity is mediated by monoclonal IgM. Anti-myelin antibodies are directed against glycoconjugates in the peripheral nerve. Glycoconjugates are present on the cell surface facing outwards and are involved in adhesion and binding with numerous ligands. They all have a common epitope called HNK-1: antibodies can therefore be directed against MAG, the P0 glycoprotein, or against different glycolipids: SGPG or SGLPG. Anti-myelin antibodies are tested by IFI on a cross-section of monkey sciatic nerve. As the activity is carried by a monoclonal IgM, only antibodies of this isotype are tested. It is less sensitive than enzymatic techniques, but has good specificity. It is therefore a screening technique that can be supplemented by testing for anti-MAG or anti-SGPG antibodies.
