Clinical significance
Aspergillus are ubiquitous, opportunistic filamentous fungi responsible for allergic reactions. They can eventually spread to the bronchopulmonary tree, the ear canal, skin and nails. Their implantation in the form of aspergillary grains is favored by a pre-existing lesion (old tubercular cavern, burn) and above all by a state of advanced immunodepression. In such cases, lesions rapidly become invasive and debilitating. Culture on a selective medium, repeated on several samples, enables the strain to be identified, but its involvement in the pathology can only be envisaged if fungal elements are found in the lesion or biological fluid in question. Circulating antigen testing enables rapid diagnosis of invasive aspergillosis in highly immunocompromised patients. Serology is useful in immunocompetent subjects with suspected deep-rooted aspergillosis, but is generally positive late in the diagnosis.
