Clinical significance

Alpha1-glycoprotein binding specifically with cortisol with a high affinity. It increases with oestrogen treatments and during pregnancy. CBG levels are diminished in Cushing's syndrome, corticoid treatments and cases of low blood protein. In cases of septic shock, very low CBG levels have been reported. The ,primary clinical significance of the CBG assay lies in its use to calculate the non-bound cortisol concentration by determining the mass matio of the total serum cortisol concentration in relation to that of CBG.
