Clinical significance
Component C4 interacts with C1 and C2 to form the C3 convertase of the classic complement activation pathway (C4b,C2a). Activated C1s cleaves C4 to generate an anaphylatoxin (C4a) and a larger fragment (C4b). Congenital C4 deficiency is known and some forms are associated with specific HLA haplotypes because the genes encoding C4 are located in the Major Histocompatibility Complex. Systemic lupus erythematosus (SLE) is often associated with congenital C4 deficiency. Reduced levels of components of the classic pathway (C1, C2, C4, C3) are common after such activation, e.g. in SLE, acute serum sickness and various different conditions associated with circulating immune complexes.
