Clinical significance
One of the CK isoenzymes found in the cytosol of cardiac muscle cells. The ratio of CK MB to total CK (normally lower than 0,06 at all ages and in both sexes) is only suggestive of heart disease if total CK levels are elevated. The result can be distorted by : - the presence of CK BB - results in overestimation (> 100UI/L); - the presence of atypical, complexed CK (macro-CK which is not inhibited by the anti-M sub-unit antibody), can indicate a CK MB activity which is actually higher than that of total CK. These problems can be overcome by a quantitative CK MB assay, by analyzing CK isoenzymes by electrophoresis or by assaying troponin Ic. Enzyme activity can increase significantly with exercise and varies between different ethnic groups.
