Clinic significance
An adrenal steroid intermediate in cortisol biosynthesis. Derived from the conversion of 17-hydroxyprogesterone by 21-hydroxylase. It is one of the 17-hydroxycorticosteroids and is useful in the diagnosis of 11-beta-hydroxylase deficiency in congenital adrenal hyperplasia and for the evaluation of anterior pituitary insufficiency (Metyrapone test). Metyrapone specifically blocks the conversion of 11-deoxycortisol to cortisol; as a consequence, the plasma cortisol concentration falls so the normal anterior pituitary secretes more corticotropin and 11-deoxycortisol levels begin to increase again.
