Clinical significance

A respiratory tract infection due to Bordetella pertussis which results in a characteristic cough (whence the popular name for the condition "whooping cough"). Can be serious in babies. Widespread immunization has reduced its incidence but the vaccine is not 100% protective. Older children, adolescents and adults can sometimes become infected with atypical symptoms - this contributes to the spread of the infection. Bacteriological analysis is problematic because the bacterium is very delicate. The pathogen can only be detected directly by IF in nasopharyngeal swab material during the first few days and, in any case, this technique is neither specific nor sensitive. PCR assay is to be preferred. Documented seroconversion with respect to pertussis toxin is very valuable for differential diagnosis but an isolated positive result could correspond to a genuine case of pertussis infection established already for several weeks.
