Clinical significance

A cardiac glycoside used to treat congestive heart failure and certain kinds of arrhythmia.70% is reabsorbed in the intestine. 20% is bound to plasma proteins. Little metabolism in the liver. Rapidly eliminated, mainly (90%) in the urine with complete clearance in 2 to 6 days. Dose should be adjusted for patients with impaired renal function.Stable concentration reached in 5 to 8 dayswith a half-life = 1.5 days.Overdosage results in gastrointestinal symptoms (anorexia, nausea and vomiting) and neurosensory problems (dizziness, headaches and yellow-hued vision) with the most serious toxicity in the heart with impaired conduction and enhanced excitability. Low blood potassium or high blood calcium can potentiate its activity.
