Clinical significance

Main glucocorticoid hormone synthesised from cholesterol in the fascicular zone of the adrenal cortex. Its secretion is controlled by ACTH with negative feedback involving CRF (Corticotrophin-releasing-factor) from the hypothalamus. Cortisol has glucocorticoid properties (effects on glucides, lipides and proteins metabolism). Urinary free cortisol, which only represents a small fraction (1%) of secreted cortisol, nonetheless provides a good reflection of the serum cortisol fraction not bound with proteins and there is a good correlation between its excretion and the cortisol secretion rate by the adrenal cortex. The measurement of its excretion is the best criterion for diagnosing hypercortisolism. The level is thus elevated in all cases of Cushing's syndrome.
