Clinical significance
The essential role of haemopexin is haematin elimination (haeme degradation product). It may also be involved in the metabolism of other porphyrins. The haemopexin level is increased in cases of haemochromatosis and rheumatismal, inflammatory and infectious diseases. However, differential intravascular haemolysis diagnosis is of major interest: - in cases of moderate haemolysis, the haptoglobin level decreases significantly while the haemopexin level decreases moderately; - in cases of severe haemolysis, the haemoglobin fixing capacity of haptoglobin is exceeded and free haemoglobin is oxidised to methaemoglobin which separates into haematin and globin. Haematin then binds with haemopexin; the free haemopexin level thus decreases significantly. In cases of haemolysis in addition to an inflammatory syndrome with normal haptoglobin, haemopexin is used to assess the severity of the haemolysis.
