Clinical significance

If C1 inhibitor (more specifically, its esterase activity) is inactive, complement is permanently activated via the classic pathway. An absolute decrease in the level of this protein (less than 30% of the normal level) accounts for 95% of hereditary forms of angioneurotic edema which is characterized by recurrent episodes of edema involving the skin and the laryngeal and intestinal mucosae.There exist also acquired forms of C1 inhibitor deficiency, e.g. secondary to lymphoma, neoplasia or, more rarely, due to the presence of a specific autoantibody.In parallel to the functional C1 inhibitor deficiency test, its functional activity should be measured and a quantitative assay should be carried out.
