Kappa or lambda free light immunoglobulin chains (FLC) can be assayed using specific antisera of epitopes normally masked when light chains are associated with heavy immunoglobulin chains. The daily FLC production in normal subjects is approximately 500 mg, from bone marrow and lymph node plasmocytes. In normal subjects, polyclonal serum FLC are filtered at a glomerular level, reabsorbed by the proximal tubule and 1 to 10 mg per day are secreted by the distal tubule mucosa. The FLC assay is performed for the diagnosis and monitoring of FLC myeloma, non-secretory myeloma and to evaluate the response to chemotherapy and for early screening of recurrent whole immunoglobulin myeloma. These assays would be an indicator of the malignant transition of monoclonal gammopathies of undetermined significance (MGUS). Due to the extent of proximal tubular uptake, urinary FLC concentrations are more dependent on kidney function than tumour synthesis.
