Clinical significance
Lysozyme is a hydrolase enzyme produced by polymorphonuclear neutrophils and monocytes/macrophages. Serum lysozyme levels more or less correspond to the numbers of granulocytes and monocytes in the blood. Only very small amounts of lysozyme are found in the urine because, although it is passed through with the glomerular filtrate, it is almost entirely reabsorbed in the renal tubules.Increased serum lysozyme levels are most commonly due to conditions in which granulocytes or monocytes are proliferating (both malignant and non-malignant conditions), i.e. acute myelogenous, myeloblastic or monocytoid leukemia are all associated with particularly high levels and chronic myelomonocytic leukemia with less dramatically elevated levels. Tubular kidney disease can also cause somewhat increased serum lysozyme. Reduced levels in the serum are associated with aplastic anemia. This test can be used to distinguish between central and peripheral etiologies for neutropenia.
