Clinical significance
Noroviruses (sometimes also called ""Norwalk viruses"") are non-enveloped RNA viruses. They belong to the Caliciviridae family. They display a high degree of genetic diversity, and their strains are classified into genogroups, which are in turn subdivided into genotypes. Genogroups I and II are the most important in humans.Noroviruses are the main agents of acute gastroenteritis in all age groups. These gastroenteritises are characterised by the sudden onset of vomiting and/or diarrhoea after a short incubation period of around 24 to 48 hours. Most infections heal spontaneously in less than 2 to 3 days. However, more severe clinical forms have been observed, particularly in immunocompromised patients. Noroviruses are mainly transmitted via the faeco-oral/interhuman route. Norovirus infections can occur in the form of epidemics. They are a major problem in medical institutions such as nursing homes and long-term care services.Norovirus antigen detection kits are available. These commercial kits are available in the form of ELISA or rapid immunochromatographic tests. These techniques may lack sensitivity, but their specificity is close to 100%. These tests have the advantage of being quick and easy to use.Specific PCR diagnosis of norovirus infection using a stool sample is rapid, sensitive and reliable, and is of major interest for fragile populations or in case of a suspected epidemic.
