Clinical significance
Idiopathic extra-membranous glomerulonephritis is one of the main causes of glomerulopathies in adults and can lead to severe or terminal renal failure. The aetiology and pathophysiology of this disease are still poorly known, but the latter is marked by the existence of immune deposits in the glomerular basal membrane, comprised of circulating antibodies targeted against an antigen present on the surface of epithelial cells essential for glomerular filtration, podocytes. Over 70% of adult patients with idiopathic extra-membranous glomerulonephritis have IgG4-type circulating autoantibodies targeted against the M-type phospholipase A2 receptor(PLA2R ) present on the surface of podocytes. These autoantibodies recognise one of several conformational epitopes of the receptor and are not present in other glomerulopathies or secondary forms of extra-membranous glomerulonephritis. Moreover, their preliminary works suggest an association between the levels of circulating autoantibodies and the clinical stage of the disease.
