Clinical significance

Protozoa and helminths are frequently associated with persistent digestive disorders when returning from a trip to a tropical region, or in immunocompromised subjects. Symptoms are often more diffuse than in acute bacterial or viral infections, and diarrhoea is not always the main feature. The spectrum of clinical manifestations of these infections is broad. Depending on the aetiology, there may be transit disorders, abdominal pain, epithelial discharge and tenesmus, among other symptoms. Transmission is indirect (water, food) or direct (hands, contact with faeces). Multiplex PCR testing is indicated in the presence of clinical digestive signs and in the absence of travel, or when travel has taken place in Western Europe. For all other clinical-biological situations, parasitological examination of the faeces using microscopy is recommended. PCR is used to detect DNA from the following parasites: 1- The helminth panel - Ancylostoma spp, - Ascaris spp, - Enterobius vermicularis, - Hymenolepis spp, - Necator americanus, - Strongyloides spp, - Taenia spp. - Trichuris trichiura and - two genera of microsporidia (Enterocytozoon spp. and Encephalitozoon spp). 2- The prorozoan panel - Blastocystis hominis, - Cryptosporidium spp, - Cyclospora cayetanensis, - Dientamoeba fragilis, - Entamoeba histolytica, - Giardia lamblia. Given the intermittent excretion of parasites, it is recommended that digestive parasites be detected in 3 separate samples taken a few days apart (PCR or microscopic examination).
