Clinical significance

A bactericidal glycopeptide antibiotic, chemically similar to vancomycin. It inhibits Gram-positive bacillus wall synthesis. It is active against all Gram-positive bacilli and Gram-positive anaerobes, but is essentially used for treating methicillin-resistant staphylococci. It is administered by the IV or IM route for systemic infections. It is eliminated in the urine rapidly and unmetabolized (i.e. in its active form). Its half-life is very long, 40 to 100 hours in adults. Teicoplanin is a ""time-dependent"" antibiotic. Its efficacy increases with the time present on the infected site. This implies that the residual concentration must be sufficiently high to obtain satisfactory efficacy. It is recommended to measure residual concentrations 48 hours and 8 days after the start of treatment. It is not necessary to evaluate the peak concentration.
