Clinical significance
The Zika virus is an arbovirus belonging to the Flavivirus genus. It is transmitted mainly to humans by the bite of an Aedes mosquito (which also transmits Dengue fever and Chikungunya). Human-to-human transmission is also possible through sexual contact. Most often asymptomatic, infection by the Zika virus can lead to an flu-like syndrome with a high fever, headaches, arthralgia, conjunctivitis and maculopapular exanthema. These symptoms are generally benign and have a favourable course. In some cases, however, the disease can be complicated by neurological symptoms such as meningitis, encephalitis and Guillain-Barré syndrome. Diagnosis is based on PCR testing of the viral genome in blood up to 7 days after the onset of symptoms, or in urine up to 10 days after symptoms have appeared. Serology can be performed from day 5 onwards. Because of the similarity of geographical distribution and clinical signs, it is recommended that Dengue fever and Chikungunya infection be investigated simultaneously. In addition to neurological complications, the risk of infection by the Zika virus is based on the possibility of congenital malformations (the most frequent neurological and ophthalmic disorders) as a result of its transplacental passage during pregnancy. With a view to medically assisted procreation or fertility preservation, the Zika virus genome may be sought in the sperm of men who have been exposed, due to its prolonged excretion. Zika virus infection is a reportable disease.
